- The CRC form can be downloaded by clicking the above link or is available at Nancy Moore's residence, 2460 Leclair Drive, Coquitlam. There is a box outside the front door with blank ones in it.
- The Letter of request must be brought along with the CRC form to the RCMP.
After RCMP processing, bring the forms to Nancy Moore, Risk management coordinator.

Another option for applying for a Criminal Record Check is an Canadian company that will process a CRC online for a fee of $25. Click here for more info www.mybackcheck.com.

Note: Form A is required for the adult women's and men's teams as well as a few under-age coaches, who will be advised separately to acquire this form.
